What You Should Know About SNP Plans in New London County
To qualify as a special needs individual you will need to demonstrate one or more of the following criteria:
- You will be institutionalized for 90 or more days,
- You are dual-eligible for both Medicare and Medicaid, or
- You have a severe or disabling chronic condition approved by Medicare.
There are three different types of SNP plans:
When You Can Join a Special Needs Plan
Even if you qualify, you are only allowed to join, drop or change plans during a Medicare Election Period. The period between 15 October and 7 December is the Annual Election Period (AEP). That's when most people in New London County make changes to their Medicare.
- D-SNP plans are for individuals who qualify for both Medicare and Medicaid (dual-eligible).
