What You Should Know About SNP Plans in Lyon County
SNP plan enrollment is strictly limited. Enrollees must demonstrate qualification through one or more of three criteria:
- An institutionalized individual,
- A dual-eligible (Medicare and Medicaid) person, or
- An individual with a severe or disabling chronic condition approved by Medicare.
Not all Medicare Special Needs Plans are the same. There are three different types:
Medicare SNP Enrollment
For those who qualify there are several different Medicare Election Periods. This is when you can join or change your plan. Most Lyon County residents wait until 15 October to 7 December to make enrollment changes. This is the Annual Election Period (AEP).
- D-SNP plans are for individuals who qualify for both Medicare and Medicaid (dual-eligible).
- I-SNP plans are for people who are institutionalized and live in long-term care or skilled nursing facility.
