What You Should Know About SNP Plans in Mercer County
SNP plan enrollment is strictly limited. Enrollees must demonstrate qualification through one or more of three criteria:
- An institutionalized individual,
- A dual-eligible (Medicare and Medicaid) person, or
- An individual with a severe or disabling chronic condition approved by Medicare.
Not all Medicare Special Needs Plans are the same. There are three different types:
When to Enroll in a Special Needs Plan
If you qualify, you can join or switch plans during a Medicare Election Period. For most people living in Mercer County, the Annual Election Period (AEP), which starts on October 15th and ends on December 7th, is the time they update their enrollment.
- D-SNP plans are for individuals who qualify for both Medicare and Medicaid (dual-eligible).
- I-SNP plans are for people who are institutionalized and live in long-term care or skilled nursing facility.
- C-SNP plans are for people with one or more chronic conditions, including diabetes, end-stage renal disease (ESRD), lung conditions, or heart disease.
